Policies

Payment

Payment must be made at the time of service. You may pay by credit or debit card. Your credit card will be charged when the test order is submitted. Charges will appear on your receipt. Please review your receipt and save a copy for your records. If you have any questions or concerns about your invoice you must contact us within 7 days to notify us of the charges you wish to dispute. If you fail to notify us within 7 days, you waive all rights to dispute charges.

We reserve the right to refer any past due account to an attorney at law.

Specimen Collection

We do not collect or ship specimens. You will need to contact Quest Diagnostics directly for assistance.

Personal Information

You agree to allow Freedom Lab Orders or any of its employees, members, and providers access to lab results in the occurrence you are unable to retrieve lab results timely or if you suspect there may be an error in the lab order, or any other circumstance Freedom Lab Orders deems necessary as an administrative role only. This, in no wise, is an excuse to have lab results reviewed, nor interpreted by the ordering provider, nor will you hold any of its providers, employees, or members responsible for reporting or interpreting lab results. You must contact your medical provider for any review, interpretation, or management of healthcare needs related to any lab orders and results.

PATIENTS WITH MEDICARE INSURANCE

IF YOU ARE A PATIENT WITH MEDICARE, YOU ARE NOT ALLOWED TO USE THIS SERVICE DUE TO AGREEMENTS BETWEEN THE LAB PROVIDER AND MEDICARE.
